Dermoid cyst & endometrioma were separately discussed …
We are taking about e.g, Simple cyst -hemorrhagic -corpus Leuteum cyct – mucinous cystadenoma- paraovarian cyst.
خد بالك من النقط الاتيه :
١- المبيض ده حاجه مقدسه … ده مستقبل البنت او حياه الست … ممنوع تشيل Ovarian tissue و انت بتشيل Cyst wall
(Of course unavoidable microscopic ovarian tissue will be removed , but macroscopic ovarian tissue should never be removed during cystectomy).
٢- قواعد التعامل مع المبيض :
– Be non-traumatic as possible.
– If you are in the right plan bleeding will be minimal , least affection of ovarian tissue.
– Adequate dissection.
– No excuse to leave any cyst wall.
– Adequate hemostasis with least use of electrosurgery.
٣- وانت بتفتح المبيض استعمل Scissors احسن بدل ما تستعمل الDiathermy … اى Diathermy هياثر على مخزون البويضات … و انت بتوسع الفتحه براحه و الات Atraumatic : GENTLE handling.
4- The only cyst that should not be ruptured as much as you can is dermoid … all other can be ruptured to facilitate cystectomy
٥- لو وصلت للـ Right plane تبقى العمليه سلسه خالص ..
Adequate plane = easier + least blood loss + least affection of ovarian reserve
6- You MUST do complete cystectomy …. remove ALL the cyst wall … exhaust yourself … be patient … there is nothing called remove some & that is enough !!!!! Or coagulate the remaining …..
ALL the cyst wall must be removed …. NEVER to ablate cyst wall with monopolar …. the only ablation is LASER/Plasma jet (still controversial).
7-Hemostasis
استنى شويه ما تستعجلش … سيب الجسم يشتغل شويه و يوقف النزيف بنفسه الا لو لقيت Active Spurtor ماشى … ما عدا كده اهدى … افتكر ان كل لسعه دياثرمى ممكن تاثر على مخزون البويضات …
Bipolar only should be used + interrupted + cooling …..
If monopolar (better avoided ) : cutting … low voltage (50 max) interrupted … the least possible …
٨- فيه ناس بتشفط الكيس … و تلسع بالmonopolar و تمشى … دول باقولهم : اتقو الله … يا ريتك ما عملت حاجه كان احسن … الكيس ١٠٠٪ هيفضل زى ما هو … و الحاله هتعمل تانى فى خلال كام شهر (بس انت ما شفتهاش لما راحت لحد تانى و بتدعّى عليك).
٩-اخيط المبيض ولا لا ؟
No actual benefit …. Even some studies say it may increase adhesions….. Whatever the opening in the ovary, it will recover again ….. Remember that during the normal ovulation 1/3 to 1/2 the ovary is opened !! …
ممكن لو الكيس كبير اوي تلم الدنيا بغرزتين مثلا … بس الغرز تبقي من جوه ما تبانش علي الوش …
١٠-Para-Ovarian cyst (السهل الممتنع )
– Cysts arising from the leaves of the broad ligament … stretching the tube … adjacent to the ovary
– Most of them diagnosed as ovarian cyst , in laparoscopy appeared to be para ovarian (lucky patient)
– تبان مغريه انك تقطعها … إياك ثم إياك … هتلاقى الانبوبه انقطعت و انت مَش واخد بالك … تعامل ككيس عادى جدا … و هى بتبقى سهله اوى … و الاناتومى بيرجع عادى جدا بعدها … مهما كانت كبيره.
